Community Outreach

Services Offered by the Community Outreach Specialist:

  • Serves unhoused or at-risk community members to ensure that they are aware of relevant resources, are connected to the appropriate services/organizations, and can access those services
  • Connects individuals to emergency shelters, transitional housing opportunities, and other organizations that also provide housing-related support or residential opportunities
  • Provides personalized assistance or advocacy for unhoused/at-risk individuals with any physical, communication, or mental health barriers so that they can access services and better their situation
  • Transportation assistance may be provided for unhoused/housing-insecure individuals so they can access critical services, such as:
    • Homeless shelters, day centers, or any agencies serving the vulnerable
    • Trips to agencies to apply for emergency financial or rental assistance
    • Medical appointments
    • Government offices, i.e. Social Services, Social Security Administration, the DMV/Tax Collector’s office, the courthouse
    • Food banks/pantries
    • Thrift stores (can connect people in need to free shopping vouchers for essential items)
    • Probation-mandated appointments to ensure a smoother re-entry
  • Supports the SAPD’s Family Reunification Program by helping individuals in need travel to reunite with their source of support
